Registration Forms
Please print and complete both the new registration form and the GMS1 form as we do require both. You can also come into the surgery to collect the forms.
Please fill in both the GMS1 form and the appropriate registration form and send them in an email to highclifferegistrations@nhs.net. Alternatively these forms can be printed, once filled in and brought into the Practice.

They cover issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.
